Well, not that you are saying this, but definitely don't do it with live rounds and don't try it with other calibers. We used these:
    http://www.brownells.com/.aspx/sid=2...Yellow__Qty_10

The round will seat in the mag (between regular 5.56 rounds) without a problem. It will also chamber, but because of it's length it will not stop the next round from chambering. Presto! Double feed. Sometimes they get a little jammed in there and took a little extra work to remove, but generally they were able to be cleared by vigorous remedial action. Sometimes becuase it's not quite a true double feed, immediate action (push, pull, rack, bang) took care of them. But it was the most reliable way I've seen to induce something more than FTF.

Most of the drills we ran interspersed 5.56 rubber dummy rounds, live rounds, and these to generate fairly unpredictable malfunctions are regular (but not constant) intervals. A sample setup might be:
    2 live rounds
    1 .40S&W dummy round
    3 live rounds
    1 5.56 dummy round
    2 live rounds

Are you doing this for yourself and by yourself, or with a range partner, or for training others?

I was at a recent training event for work and what we did was take multiple rifles and deliberately set them up for each of the specific malfunctions that can occur, either due to a mag issue or worn extractor issue.

Without you being present, or without you looking, have your co-workers/buddies set up multiple rifles, some with failures, some without, face them ejection port down on the ground. After he is done, he'll have you pick it up and engage a target downrange. PING!? Excellent. Click or nothing? Diagnose.

We are LE and we did this whole training with live rounds (it was part of a carbine course). If you are not comfortable with doing that, then adjust accordingly.

Take a slight amount of material out of the magazine catch notch on the side of the magazine. This would cause a bolt over malfunction. You could always put the magazine spring in backwards and cause a bullet nose down misfeed malfunction.

Cutting some of the coil off the magazine spring can help induce some malfunctions.
