If you do not get the spring seated deep into the extractor groove with the needle nose pliers, then reassembly is really, really tight.
I just went through this with three different AR's. If the spring wasn't seated then the extractor just held the round and would not release.
When seated everything was great. Just my .02
I run an "upgraded" extractor setup in my 16" middy here.
I don't "need it" most of the time - but in times where I have to put up a high volume of fire - I've never had a FTE.
Before the upgrade on a few courses etc. once the gun got hot it would have FTE's.

Kevin...
Interesting.
I use an LMT MRP 16" Mid Length, and when I put the heavy duty spring, donut and black insert in the gun, it would eject the rounds only about 1 foot away at 3:00. Pretty much landing just next to my right foot (hardly getting out of the ejection port, and some wouldn't extract at all. But I didn't replace the EJECTOR spring which had about 4,000 rounds on it, which may have been the issue.
With just a Mil Spec. Spring and a Black insert, the brass ejects about 7-10 feet away, at about 3-4:00 to the right....every time. And yes, I did put a new EJECTOR spring in the bolt.
I could go back and try the upgrade lit again now that I put the new Ejector spring in, but the gun isrunning so welll,,,AGAIN, that I don't want to mess with success.
Tack
I've got one standard length carbine gas system and one mid length. Sounds like I can upgrade both with one kit.
Stiff spring in the mid length and the black insert & O ring for the short one. I'll be damned if I run one with an ejector so stiff it won't chamber properly. In that case something is going to wear that is not intended to.
