    As I read block A, it states a written request and authorization prior to ANY interstate travel.

    I too live minutes from the state line and do most of my shooting in the other state. I filled out a form 20 for each of about 10 addresses or locations where I shoot or visit and mailed them in duplicate. I had them back in hand in 12 days. All of them have a date range of 1 year.

    I decided to submit forms for places I visit as well, such as my parent's place, in-laws and friends homes. No shooting there, but just wanted to cover all bases.

    The forms should go to your examiner who signed off on your NFA item(s). I think it was Robb or Rob who said that he needed forms in a hurry for a class one time and his examiner expidited the process for him. If you need something, or have questions don't hesitate to call them. They have been very helpful in answering any questions I have had in the past.
