    I think of two potential causes: 1st, the extension indexing pin is not in it's slot on the upper receiver. If that is the case, no worries because you don't need to remove the FSB entirely from the barrel to remove the barrel or tighten the barrel nut. All you need to do is pop out the taper pins, slide the FSB forward, unbolt the 6 bolts on the lite rail, slide the rail forward and go about the normal procedure for removing and installing the barrel.

    Or worse, the second cause would be they did something wrong and sheered off the indexing pin by significantly over torquing the barrel nut. That would be a bit more problematic.

    As others have said that looks pretty far off and I'd be calling them to complain as well. That should have been caught before it got sent back to you.

    Please update us with the shop/vendor and what they do to try and make it right.

    No it isn't right and yes you should contact them and tell them to make it right. On the other hand I would be hesitant to do so since they obviously screwed the pooch once.

    No I'm not actually. However, this upper is brand new and it's a BCM. It is possible but highly unlikely.
